Aviation Heritage Speakers Program
Investigating and Analysis of B-24 San Francisco Peaks Crash Location
Date and Time: Thursday, April 18, 2024, starting at 18:00 Arizona
Speaker(s): William Waldock, Professor of Safety Science, Embry-Riddle Aeronautical University, Prescott, Az
Brief Description:

This 1-hour seminar will focus on the wreckage and the 10 trips the International Society of Air Safety Investigators made to a B-24 crash site from 1993 -2016 to analyze the crash, and how the site has evolved over the years.  Professor Waldock will explain how the crash happened and aspects of the crew members.  The B-24 crew was on a night navigation mission from Kirtland Field, Albuquerque to Bakersfield, CA and back.  During the dark, moonless, night the crew flew off course and ended up 15 miles north of where they were supposed to be.  Cruising at 11,000 ft they flew directly into the side of Humphrey's Peak (12,633ft high).  Some of Professor Waldock's research will provide aspects of the personal effects the crash had one of the student pilot's family members.  Lack of  maintaining situational awareness is a major contributor to CFIT and definitely part of the crash.

The objective of the WINGS—Pilot Proficiency Program is to reduce the number of accidents in General Aviation (GA) by assisting airmen to find educational opportunities designed to help them apply the principles of risk assessment and risk management (RM). When properly applied, these principles will help mitigate accident causal factors associated with common pilot errors, lack of proficiency, and faulty knowledge. The Federal Aviation Administration’s (FAA) purpose is to encourage the majority of GA pilots, through WINGS, to engage in ongoing, targeted flying tasks and learning activities keyed to identified risks and which are designed to mitigate those risks.

CFIT is defined as an unintentional collision with terrain (the ground, a mountain, a body of water, or an obstacle) while an aircraft is under positive control. Most often, the pilot or crew is unaware of the looming disaster until it is too late. CFIT most commonly occurs in the approach or landing phase of flight.  In a typical year, there are about 40 CFIT accidents, about half of which are fatal.

Pilot airspace deviations continue to be a major risk in the national airspace system. To reduce the chance that you might be involved in an airspace deviation incident, you are highly encouraged to take an on-line learning center course on http://www.faasafety.gov/. Or search the learning library for associated guidance on airspace incursions. Remember, safe and prudent airmen have superior situational awareness and have a plan to keep it that way.
